Regional Research Challenge Online Entry & Required Paperwork Instructions

Entry Form Day (Turn Forms in Person): FEBRUARY 16, 2010

In order to ensure that your application is successful, please follow the procedure below. Your application cannot be accepted by the Central NM Science & Engineering Research Challenge until all of these steps are completed. BE SURE YOU KEEP COPIES OF ALL PAPERWORK, FORMS, etc.

Fill out and submit the ONLINE ENTRY FORM. PLEASE BE SURE TO CHECK ON THE NUMBER OF SLOTS YOUR SCHOOL IS ALLOTTED - Call Laura at 277-4916. This form is activated in December each year.

• Once you have submitted your ONLINE ENTRY FORM, print the SIGNATURE PAGE and get the required signatures (Student, Parent or Guardian, and Sponsoring Teacher).

• You next need to mail OR drop off (it's best if these hard copies are dropped off at our office BEFORE the Entry Form/Paperwork Deadline listed above or AT the Entry Form Day on February 16, 2010 between 2:30pm-7:00pm (see location information below). This will avoid any possibility of paperwork getting lost in the mail.

  A REGIONAL ENTRIES TRANSMITTAL FORM (PDF) which provides us with a complete listing of all students competing at the Central NM-SERC from a specific school. Here's a SAMPLE TRANSMITTAL FORM in case you need to see exactly what goes where! PLEASE USE THE LATEST VERSION OF ACROBAT READER or ADOBE ACROBAT TO OPEN THE FILE. You will be able to type directly into the form. PRINT OUT THE COMPLETED FORM IF YOU ARE NOT ABLE TO SAVE IT!

  The printed SIGNATURE PAGE (PDF) with the required signatures (this is the media release & emergency release).

  If your school/group had its own Scientific Review Committee (SRC) and/or Institutional Review Board (IRB) that reviewed human subjects and potentially hazardous biological agents projects for PRIOR APPROVAL, please complete the LOCAL SRC/IRB MEMBERSHIP FORM and include it with your Transmittal Form.   If you have the current version of Acrobat Reader (9.0 or higher - available FREE at http://www.adobe.com), the forms can be filled in on your computer (but NOT saved unless you have the full Adobe Acrobat program, so print it BEFORE you close it!), printed, signed, and sent or brought in with the remainder of your school's regional entries.

  A $17.50 entry fee per student (or a PO number from the school can be included on the Transmittal Sheet). Please note that the fee is going up for the 2009-2010 school year as a result of increasing costs of operation.

  All 2010 ISEF FORMS - REQUIRED FOR ALL PROJECTS. These forms can be typed into if you have the latest version of Acrobat Reader or a full version of Adobe Acrobat. Please note that Acrobat Reader will NOT let you save forms that you've typed information into, so be sure to print out your completed forms!

  ADDITIONAL 2010 PROTOCOL FORMS that may be needed for your project (ex: Human Subjects, Potially Hazardous Biological Agents, Vertebrate Animals, Hazardous Chemicals/Devices, etc.) are below:

  Please be familiar with the 2010 ISEF RULES & REGULATIONS (PDF). This will help avoid student disqualification resulting from failure to comply with ISEF rules.

IMPORTANT NOTE: Many of the forms must now be submitted AND approved by an IRB or SRC BEFORE research can begin. Please plan accordingly!

If submitting protocols to the Central NM Science & Engineering Research Challenge's SRC, please plan to get your protocols in as early as possible. It could take several weeks to complete the approval process depending upon the level of complexity of your project.


FINAL REGIONAL SRC/IRB/IACUC PROTOCOL DEADLINE: December 18, 2009 Please try to get protocols that need prior approval by regional compliance reviewers to us AS EARLY AS POSSIBLE...Don't wait until the last minute! (This is for protocols that need to be reviewed by the CNM-SERC Scientific Review Committee because the school does not have its own SRC as well as ALL Vertebrate Animal projects.)

ALL Vertebrate Animal projects MUST be reviewed by the UNM Animal Care & Use Committee BEFORE data collection begins.
