Entry Form Instructions
Entry Form Mail Deadline: February 19, 2008 (received in our office by this date)
Entry Form Day (Turn Forms in Person): February 19, 2008
In order to ensure that your application is successful, please follow the procedure below.
Your application cannot be accepted by the Central NM Science & Engineering Research Challenge until all of these steps are completed.
- Fill out and submit the ONLINE ENTRY FORM. PLEASE BE SURE TO CHECK ON THE NUMBER OF SLOTS YOUR SCHOOL IS ALLOTTED - Call Laura at 277-4916.

Once you have submitted your ONLINE ENTRY FORM, print the SIGNATURE PAGE and get the required signatures (Student, Parent or Guardian, and Sponsoring Teacher).
- You next need to mail OR drop off (it's best if these hard copies are dropped off at our office or at the Entry Form Day on Tuesday, February 19, 2008 between 2:00pm-7:00pm (see location information below). This will avoid any possibility of paperwork getting lost in the mail):
A 2008 SCHOOL TRANSMITTAL SHEET (PDF) which provides us with a complete listing of all students competing at the CNM-SERC from a specific school.
The printed SIGNATURE PAGE (PDF) with the required signatures (this is the media release & emergency release).
If you have the current version of Acrobat Reader (7.0 or higher), the form can be filled in on your computer (but not saved unless you have the full Adobe Acrobat program), printed, signed, and sent in.
A $15.00 entry fee per student
(or a PO number from the school can be included on the Transmittal Sheet)
All 2008 ISEF FORMS - REQUIRED FOR ALL PROJECTS. These forms can be typed into if you have the latest version of Acrobat Reader or a full version of Adobe Acrobat. Please note that Acrobat Reader will NOT let you save forms that you've typed information into, so be sure to print out your completed forms!
Any ADDITIONAL 2008 PROTOCOL FORMS for your project (ex: Human Subjects, Potially Hazardous Biological Agents, Vertebrate Animals, Hazardous Chemicals/Devices, etc.).
Please be familiar with the 2008 ISEF RULES & REGULATIONS (PDF).
This will help avoid student disqualification resulting from failure to comply with ISEF rules.
IMPORTANT NOTE: Many of the forms must now be submitted AND approved by an IRB or SRC BEFORE research can begin. Please plan accordingly!
If submitting protocols to the Central NM Science & Engineering Research Challenge's SRC, please plan to get your protocols in as early as possible.
It could take several weeks to complete the approval process depending upon the level of complexity of your project.
FINAL REGIONAL SRC/IRB/IACUC PROTOCOL DEADLINE: December 17, 2007 (This is for protocols that need to be reviewed by the CNM-SERC Scientific Review Committee because the school does not have its own SRC as well as all Vertebrate Animal projects.
ALL Vertebrate Animal projects MUST be reviewed by the UNM Animal Care & Use Committee BEFORE data collection begins.
